Neers Test

Neer’s test is a simple exam that assesses if your shoulder pain and limited range of motion may be caused by an impingement (pinching of tissue). Your healthcare provider or physical therapist can perform the Neer impingement test as a part of a comprehensive shoulder examination.

What is a positive impingement test?

The examiner places the patient’s arm shoulder in 90 degrees of shoulder flexion with the elbow flexed to 90 degrees and then internally rotates the arm. The test is considered to be positive if the patient experiences pain with internal rotation.

What does the Hawkins Kennedy test determine?

The Hawkins Kennedy test is used to assess shoulder impingement. In this test the clinician stabilizes the shoulder with one hand and, with the patient’s elbow flexed at 90 degrees, internally rotates the shoulder using the other hand. Shoulder pain elicited by internal rotation represents a positive test.

What does a positive neers and Hawkins test indicate?

A positive test is indicated by pain in the anterior or lateral shoulder when in full flexion. It is indicative of problems involving the supraspinatus and the long head of the biceps brachii tendons.

What does a positive Apley scratch test indicate?

The Apley scratch maneuver is a good test of global shoulder joint function, . A positive test is present when there is compression of the subclavian and axillary vessels or the brachial plexus. It is common for the patient’s dominant arm to have less mobility due to increased muscle stiffness.

What does a positive Gerber’s lift off test indicate?

The testing movement involves the patient performing internal rotation (IR), by lifting the hand off the back while the examiner places pressure on the hand. The test is considered to be positive if the patient cannot resist, lift the hand off the back or if she/he compensates by extending the elbow and shoulder.

What is the belly press test?

The belly-press test is used to isolate the subscapularis muscle, to test the subscapularis muscle for tear or dysfunction. It is often used as an alternative to the lift-off test, when the lift-off test can’t be performed because of pain or limited internal rotation range of motion of the shoulder.

How is impingement diagnosed?

Magnetic resonance imaging (MRI ) and ultrasound can show tears in the rotator cuff tendons and inflammation in the bursa. A diagnosis of impingement syndrome may be made if a small amount of an anesthetic (painkiller), injected into the space under the acromion, relieves your pain.

How is impingement syndrome diagnosed?

To diagnose SIS, a doctor will review your symptoms and perform a physical exam. X-rays or an MRI of the shoulder can also be useful. An MRI may show fluid or inflammation in the area. In some cases, partial tearing or other damage involving the rotator cuff and surrounding structures are visible.

Do I need surgery for shoulder impingement?

Severe cases of impingement may require surgery to remove the pressure and create more space for the rotator cuff. The most common procedure for treating impingement is subacromial decompression.

What is a positive bear hug test?

The test is positive if the patient could not maintain the position of the hand against the shoulder or showed weakness in resisted internal rotation greater than 20% compared to the opposite side.
