The UPSIT involves 40 microencapsulated odors in a scratch-and-sniff format, with 4 response alternatives accompanying each odor. The patient takes the test alone, with instructions to guess if not able to identify the item. Anosmic patients tend to score at or near chance (10/40 correct).
How do you perform a UPSIT test?
The scents are released using a pencil. After each scent is released, the patient smells the level and detects the odor from the four choices. There is an answer column on the back of the test booklet, and the test is scored out of 40 items.
How do you test for olfaction?
The patient places an index finger over one nostril to block it (e.g., right index finger over right nostril). He or she then closes the eyes. Instruct the patient to sniff repetitively and to tell you when an odor is detected, identifying the odor if recognized.
