Erections become painful when they persist. A painful erection is not normal, and this condition is called priapism. In priapism, beyond the pain, the most important painful erection symptom is the duration of the erection. An erection that lasts for more than 4 hours is a medical emergency and needs prompt attention.
What causes pain in manhood?
Penis pain has many causes. For example, the penis can be injured during sports or a fall. Strenuous sex or long periods of sexual activity can also cause pain. In some cases, the pain is caused by an infection like a urinary tract infection (UTI) or a sexually transmitted infection (STI).

When to see a doctor
If you have an erection lasting more than four hours, you need emergency care. The emergency room doctor will determine whether you have ischemic priapism or nonischemic priapism. If you experience recurrent, persistent, painful erections that resolve on their own, see your doctor.
