Open “Phone” Tap “Menu” > “Settings” Go to “Call forwarding” Select the forwarding option you want to turn off and tap “Disable”

Why is my phone saying conditional call forwarding active?

"Conditional Call Forwarding Active" shows when forward when busy, forward when unanswered, or forward when unreachable is selected. To make the message go away you'd have to disable the three forwarding options in their settings.
