Isoprenaline should generally be started at the lowest recommended dose. This may be gradually increased if necessary while carefully monitoring the patient. Doses sufficient to increase the heart rate to more than 130 beats per minute may increase the likelihood of inducing ventricular arrhythmias.

Isoprenaline should be used with caution in patients with ischaemic heart disease, diabetes, hypertension and hyperthyroidism. Arrhythmias, sweating, tremor, headache and flushing.
How do I start Isoprenaline infusion?
Isoprenaline should be started at a low dose (2 micrograms/min) and titrated slowly by 1-2 micrograms/min every 2-3 minutes until satisfactory response is achieved – there is no minimum heart rate target and low doses are often all that is required to prevent compromising bradycardia or asystole.
