Trendelenburg Sign

Trendelenburg Sign

The Trendelenburg sign is said to be positive if, when standing on one leg (the ‘stance leg’), the pelvis severely drops on the side opposite to the stance leg (the ‘swing limb’). The muscle weakness is present on the side of the stance leg.

What is the Trendelenburg test used for?

The Trendelenburg test is used to evaluate for weak or injured gluteus medius and minimus muscles. It begins with the patient in a neutral stance. The physician is seated behind the patient with his or her hands placed behind the patient’s hips with the thumbs resting on the posterior superior iliac spine.

Why does Trendelenburg occur?

Trendelenburg gait occurs when your hip abductor muscles are very weak. This muscle group includes the gluteus medius and gluteus minimus muscles, located in the area of your butt. When these muscles are not strong enough, your pelvis will droop on the opposite side of the body from the affected muscles.

How is Trendelenburg measured?

The test is negative when the hip of the leg that is lifted, will also go up i.e., hiking of hip or the pelvis tilts upwards. The test is positive, when there is a drop of the hip or a downwards tilt of the pelvis. X-ray is the best way to diagnose or confirm the trendelenburg pathology.

What is a Trendelenburg bed?

The Trendelenburg position is a feature integrated into most profiling and adjustable beds. The position places the person’s head down and elevates the feet, so their whole body is sloping down with the feet higher than the rest of the body.

What is Trendelenburg test for venous insufficiency?

The Trendelenburg test is used to determine the site of valvular incompetence in a patient with varicose veins. Ask the patient to lie down. Elevate the leg, and empty the veins by massageing distal to proximal. Using a tourniquet, occlude the superficial veins in the upper thigh.

What is Trendelenburg gait pattern?

Trendelenburg gait is an abnormal gait resulting from a defective hip abductor mechanism. The primary musculature involved is the gluteal musculature, including the gluteus medius and gluteus minimus muscles. The weakness of these muscles causes drooping of the pelvis to the contralateral side while walking.

What is gluteus minimus?

Gluteus minimus is the smallest muscle of the glutei. It shares many similar features that of the gluteus medius, including structure and function, blood supply, and innervation. It is located just beneath the gluteus medius muscle. Gluteus minimus predominantly acts as a hip stabilizer and abductor of the hip.

Can you fix Trendelenburg gait?

Trendelenburg gait can be disruptive, but it’s often treatable with special shoes or exercises designed to strengthen your hip abductor muscles. If an underlying condition, such as osteoarthritis or muscular dystrophy, is causing this gait, your doctor will help you develop a treatment plan.

What is the meaning of Trendelenburg?

[ trĕn′dl-ən-bûrg′ ] n. A supine position with the patient inclined at an angle of 45 degrees; so that the pelvis is higher than the head, used during and after operations in the pelvis or for shock.

Why is Trendelenburg contraindicated?

Patient Positioning

Adverse outcomes in healthy patients have not been noted, although the Trendelenburg position is clearly contraindicated in patients with increased intracranial pressure. Lengthy procedures may result in significant facial and upper airway edema.

What is Trendelenburg position in pregnancy?

In the Trendelenburg position the body is laid flat on the back (supine position) with the feet higher than the head, in contrast to the reverse Trendelenburg position, where the body is tilted in the opposite direction. This is a standard position used in abdominal and gynecological surgery.
