The 2021 description of CPT 99211 was as follows: “Office or other outpatient visit for the evaluation and management of an established patient, that may not require the presence of a physician or other qualified health care professional. Usually, the presenting problem(s) are minimal.

What modifier can be used with 99211?

Effective for claims processed on or after March 16, 2020, we will deny reimbursement for evaluation and management (E&M) services billed with Current Procedural Terminology (CPT®) code 99211 appended with modifier 25 when billed alone or with another procedure code on the same date of service.
