Epi Drip

Epi Drip

Epinephrine is used for profound refractory hypotension, ventricular fibrillation, VT, PEA, and asystole. It is the first drug of choice for cardiac arrest. The patient is transferred to the ICU, and the hospitalist in the ICU orders an epinephrine drip to start at 2 mcg/min.

What is IV epinephrine used for?

Epinephrine injection is used along with emergency medical treatment to treat life-threatening allergic reactions caused by insect bites or stings, foods, medications, latex, and other causes. Epinephrine is in a class of medications called alpha- and beta-adrenergic agonists (sympathomimetic agents).

What should I start with EPI drip?

Start the epinephrine infusion at 0.1 mcg/kg/minute using a programmable infusion pump while continuously monitoring the patient’s cardiac rhythm and blood pressure (ie, approximately 6 to 10 mcg/minute in most adults).

When should I use an Epi drip?

IM epinephrine is usually administered to “stable” patients exhibiting signs of anaphylaxis. Guidelines recommend to “[a]dminister IV epinephrine if anaphylaxis appears to be severe with immediate life-threatening manifestations,” and starting a drip between 1-4 mcg/min.

How often can EPI be given?

You may repeat the injection every 5 to 10 minutes as needed. However, the dose is usually not more than 0.3 mg per injection.

What does epinephrine do to the heart?

Epinephrine, when injected into an intravenous fluid solution, will increase the coronary artery pressure thereby promoting increased coronary blood flow. Increased doses of epinephrine quicken the response, but some studies have shown that brain and heart damage are some of the side effects.

Does epinephrine increase heart rate?

Epinephrine, also called adrenaline, has powerful effects on the body. These include: increased blood sugar levels. increased heart rate.

Why is EPI used for anaphylaxis?

Epinephrine is the medication of choice for the first-aid treatment of anaphylaxis. Through vasoconstrictor effects, it prevents or decreases upper airway mucosal edema (laryngeal edema), hypotension, and shock. In addition, it has important bronchodilator effects and cardiac inotropic and chronotropic effects.

What is the side effect of epinephrine?

Common adverse reactions to systemically administered epinephrine include anxiety, apprehensiveness, restlessness, tremor, weakness, dizziness, sweating, palpitations, pallor, nausea and vomiting, headache, and respiratory difficulties.

How long does IV epinephrine last?

Following intravenous injection, epinephrine is rapidly cleared from the plasma with an effective half-life of

How fast do you administer IV epinephrine?

Adults – Start the IV epinephrine infusion at 0.1 mcg/kg/minute (range: 0.05 to 0.2 mcg/kg/minute) and increase it every two to three minutes by 0.05 mcg/kg/minute until BP and perfusion improve.

How do you dilute epinephrine for IV?

Stability / Miscellaneous
Dosing (Adults): Administration. Preparation of adult I.V. infusion: Dilute 1 mg in 250 mL of D5W or NS (4 mcg/mL); administer at an initial rate of 1 mcg/minute and increase to desired effects; at 20 mcg/minute pure alpha effects occur.Extravasation management: Use phentolamine as antidote.

What is the concentration of an epi drip?

The most common way to mix the infusion is to add 1mg of Epinephrine to 1L of D5W or normal saline. This gives you a concentration of 1 mcg/ml.

Is atropine the same as epinephrine?

Unlike atropine, epinephrine stimulates the entire myocardium (atria, SA node, AV node, and ventricles). As such, epinephrine may be effective in a broader range of bradycardias compared to atropine: Atropine-responsive bradycardias due to excessive parasympathetic tone can generally still be overcome by epinephrine.

Can you give EPI for bradycardia?

Epinephrine. Epinephrine infusion may be used for patients with symptomatic bradycardia or hypotension after atropine or pacing fails (Class IIb). Begin the infusion at 2 to 10 μg/min and titrate to patient response.

Is Epi a pressor?

Epinephrine is the only first-line vasopressor that also has beta-2 stimulation, leading to bronchodilation. This, combined with the alpha stimulated vasoconstriction for hypotension, means it is the recommended agent for anaphylactic shock [8].

What is the action of epinephrine?

Through its action on alpha-1 receptors, epinephrine induces increased vascular smooth muscle contraction, pupillary dilator muscle contraction, and intestinal sphincter muscle contraction. Other significant effects include increased heart rate, myocardial contractility, and renin release via beta-1 receptors.
