Positive findings may include excessive gapping at the medial joint and/or pain, indicating MCL damage. This may also indicate capsular or cruciate ligament laxity, depending on what degree of knee flexion the test is performed at. The terms valgus and varus refer to angulation (or bowing) within the shaft of a bone or at a joint in the coronal plane. It is determined by the distal part being more medial or lateral than it should be. Whenever the distal part is more lateral, it is called valgus.

A varus deformity is an excessive inward angulation (medial angulation, that is, towards the body’s midline) of the distal segment of a bone or joint. The opposite of varus is called valgus. EX: Varus deformity results in a decreased Q angle of the knee joint.
What tests will be positive when an athlete suffers an ACL tear?
Is there a test that can be done to check for an ACL tear? Yes. The best test is called the Lachman Test, which checks displacement of the tibia on the femur with the knee flexed 30 degrees. The physician also will determine if there is a solid, soft or no end-point of the ligament when doing the test.

To perform this test, place the knee in thirty degrees of flexion. While stabilizing the knee, press firmly against the outside portion of the knee while holding the ankle stable. If the knee gaps on the inner portion of the joint greater than normal (compare with the uninjured leg), the test is positive.
