Requesting a PHA (Soldiers Serviced by an MTF)
To schedule your annual DoD PHA, you must complete your online PHA assessment by going to and under “Self Service” there will be a link for “Periodic Health Assessment.”
How do you complete a PHA?
STEP 1: The first part of the PHA process is unchanged: service member completes an online questionnaire via Army Knowledge Online (AKO). To complete the online questionnaire, Soldiers should visit and then go to “Self Service” and then click on “My Medical Readiness.”
Where can I find my Medpros?
MEDPROS also provides every Soldier with alerts as to their Individual Medical Readiness status. If you have difficulty accessing or using this system, contact MEDPROS at (877) 256-6477 or via email at . Hours of operation: 0600-1800 EST Mon-Fri, 0700-1700 Sat-Sun.
Where can I find my Army medical profile?
Go to AKO home page, right column under ‘My Professional Data’ and click on ‘My Medical Readiness Status’. 2. Under “Medical Readiness Status” go down to DLC (Deployment Limiting Conditions) and click ‘view detailed information’. This will open up the ‘Medical Readiness Profile for RANK FIRSTNAME LASTNAME’.
Does a PHA include a drug test?
The Personal Health Assessment (PHA) provides a snapshot of your health through laboratory screenings, medical history and physical factors. The PHA is not a drug test.
How long does a PHA take?
The entire process generally takes approximately 7-10 minutes. Please allow an additional 5 minutes if you have not completed the PHA online questionnaire.
How do I get my Army medical records?
